I promise that the information provided in this employment application (and accompanying resume, if any) is true and complete. I understand that any false information or omissions may disqualify me from further considerations for employment, and may be justification for my dismissal from employment by Extra Step Cleaning LLC if discovered at a later date. I agree to immediately notify Extra Step Cleaning LLC if I should be convicted of a crime, other than a routine traffic matter, or be charged with any crime while my job application is pending, or during my period of employment, if hired. A conviction or charge is not an automatic bar to employment but it must be disclosed.
I authorize any person, school, current employer (except as previously noted), past employer(s), government or investigative agencies, and other organizations that may be named in this application form (and accompanying resume, if any) to provide the company with relevant information and opinion that may be useful to Extra Step Cleaning LLC in making a hiring or retention decision, and I release such persons and organizations from any legal liability in making such statements.
If offered a job that requires it, I understand that I may be asked to take any number of pre-employment tests, I also agree to submit a post-offer drug test and a job-related complete physical examination, and I consent to the release to the Company or its medical designee of any medical information they deem necessary in assessing my capability to perform the essential functions of the work for which I am applying (with or without a reasonable accommodation).
I understand that, if hired, I may not hold other employment, nor engage in consulting, sales, investments or other activities that may create a conflict of interest with Extra Step Cleaning LLC.
I understand that if employed and my employment is terminated by Extra Step Cleaning LLC for dishonesty, breach of trust, or any criminal acts, the authorities may be notified and I may be criminally prosecuted.
I understand that this application does not, by itself, create a contract of employment. I understand and agree that, if hired, my employment is for no definite period of time, and may be terminated at any time with or without cause. I understand that only the President of Extra Step Cleaning LLC is authorized to change any of the terms of employment and that any changes must be specific and in writing.
If employed, I will be required to complete an Employment Verification Form (I-9) and within three days show satisfactory evidence of identity and eligibility for employment.
I hereby authorize Extra Step Cleaning LLC (hereafter “the Company” or “Employer), its employees, agents, private investigators or any representative of the aforesaid company, to perform investigations into my background, past behavior, to my character, general reputation, and mode of living. In addition, I further authorize investigations of the following:
Investigative Consumer Reports: I authorize the Company to perform investigative consumer reports that may include credit reports, criminal history or arrest records, workers’ compensation histories, motor vehicle records, employment and unemployment records, military records, or other sources of information.
Education: I authorize schools, colleges and all scholastic institutions to release any and all information requested. This includes transcripts, grades, attendance records, and any other information requested.
Employment: I authorize all former and current employers to release any and all information regarding my employment history. This includes all information contained in my personnel file, salary history, condemnations, and all other pertinent information. I further authorize my supervisors and other work associates to disclose their opinions and observations of my work habits, qualities, competency, and skills.
Authorization to Release: I authorize custodians of the records of any agency, government agency, or company as described above to release such information upon request of any investigator, agent, or representative of the Company. I understand that any or all of these investigations or inquiries can be performed prior to and periodically throughout the duration of my employment.
Re-disclosure: I understand that the information requested is for the use by the Company and may be re-disclosed only as authorized by law. I understand that I have the right to request from the Company a written disclosure of the nature and scope of the investigation conducted that I have authorized above.
Indemnification: I indemnify, release, and hold harmless the Company, any agents of the Company , or others reporting to and for the Company, any investigators, all former employers, reporting agencies, and all those supplying references and character references, from any and all claims, defamation, demands, and for liabilities arising out of, or related to, such investigations, disclosures, or admissions.
Signature: By checking the box on the bottom of the online application form and clicking the Submit button, you are authorizing Extra Step Cleaning LLC to complete processing of the application as stated in its entirety on the application and within these Employment Terms. Your submission is your written signature giving us authorization to process your application.
